The social determinants of a person’s life can significantly impact on a person’s mental health and wellbeing. These factors include housing, education, training, employment, social support networks, social exclusion and stress. If someone is experiencing difficulties in one or more of these life areas it can create and exacerbate mental health symptoms.
It is also evident that simple lifestyle changes can make a positive difference to people’s experience of anxiety and depression. Eating well, exercising regularly, sleep and stress reduction will go a long way in alleviating the effects of these disorders.
As a practioner you may see clients presenting with a mental illness or distress who also have a number of complex social issues and stressors that are contributing to this. Or you may be prescribing lifestyle changes that your client is struggling to follow due to low mood and a lack of motivation. As much as you would like to spend more time with clients to support them with these issues, it takes time and hands on approach that is just not possible in a busy practice.

Our Wellness Coaches are qualified trained mental health support workers based in the community.
We can work with people in their own home and provide support packages that include the
following
- Understanding mental health
- Creating a wellness plan
- Managing daily tasks
- Relaxation and mindfulness techniques
- Dealing with distressing emotions
- Developing an exercise routine
- Nutrition and healthy meal planning
- Employment support including a tailored wellness plan
- Education support including a tailored wellness plan
- Utilising community resources
- Establishing relationships and social connections
- Extra support current support people including respite
Support Stream sits under Kāhui Tū Kaha. Kāhui Tū Kaha is a stand-alone Charitable Company. We have been providing a comprehensive range of community mental health services in the Auckland region since 1973.
We charge a competitive hourly rate that the clients can pay directly. People can also access Support Stress by applying for Disability Allowance if they meet the criteria. They just need to complete the Disability Certificate – Counselling Form (TBA). We welcome collaboration, advice and support from our client’s lead clinician. We believe this wrap around holistic approach will create optimal outcomes for clients.
